Obtaining Official Crash Reports For

2.  Include a $3.25 check or money order made payable to "DPS-MSHP"
3.  Mail the request and payment to:

NOTE:  You must wait 10 days after the date of the accident before requesting the official crash report.



Boone County Sheriff Department
1.  Write a written request (example form here)
2.  Inclue a $3.00 check or money order made payable to "Boone Co. Sheriff"
3.  Mail the request and payment to:

NOTE:  The Boone County Sheriff Department allows you to walk into the station to pick up the report instead of waiting for them to mail it to you (view Map).

2.  Inclue a $2.00 check or money order made payable to "Columbia Police Dept."
3.  Include a self-addressed, stamped envelope
4.  Mail the request, payment, and return envelope to:
